Step 1: Submit Notice of Retirement or Disability Retirement Application to Benefits Division

Step 2: Submit Immigration Affidavit

Step 3: Elect Survivor Option (Waivers Required if Married & Retiring Under 4022 Alternate Benefit and/or Married & Electing Single Life Benefit)

• Consolidated Retirement Plan Survivor Benefit Waiver-Spousal Consent Form (PDF)
• 4022 Retirement Plan Survivor Benefit Waiver-Spousal Consent Form (PDF)

Step 4: Complete Retirement Application

• Application for Retirement Benefits (PDF) (note: Benefits Division will provide "ready to sign" version and a health insurance addendum if eligible)

Step 5: Complete Direct Deposit and Tax Forms 

• Direct Deposit Form (PDF)
• Federal Withholding Certificate for Pension or Annuity Payments (PDF)
• State of Georgia Withholding Certificate for Pension or Annuity Payments (PDF)
